Small Claims Forms

These are the most commonly used forms.

IMPORTANT! These forms are provided in PDF (Portable Document Format) and REQUIRE the FREE Adobe Acrobat Reader Software.

You can type directly onto the forms and print copies when you are done. The "print" and "clear this form" buttons are at the bottom of the last page of each form. Be sure to print the page before you clear the form. You can also print out a blank form and complete it with an ink pen or typewriter.

Forms to use before your case

Plaintiff's Claim (SC-100): Use this form to file a small claims case. You are the Plaintiff and the person or business you are suing is the Defendant. For more information see How to Sue in Small Claims Court. You can also file your case online.
Other Plaintiffs or Defendants (SC-100A): Use this form if you are filing a case that involves three or more Plaintiffs or Defendants. Fill out this form and attach it to your Plaintiff's Claim form.
Defendant's Claim (SC-120): Use this form to sue the person suing you. For more information see Small Claims Defendant’s Information.
Other Plaintiffs or Defendants (SC-120A): If you are countersuing three or more Plaintiffs, fill out this form and attach it to your Defendant's Claim form.
Proof of Service (SC-104): This form must be filled out and signed by the person who serves your court papers. The Proof of Service tells the court that the Defendant got the court papers.
Subpoena For Personal Appearance and Production of Documents (SC-107): Get witnesses and documents to court. For more information see Subpoenas.
Authorization to Appear (SC-109): At the time of the hearing, if you are appearing for a business, someone in the military, someone in jail, or need to appear on the behalf of someone, fill in this form and give it to the courtroom clerk.
Request for Court Order and Answer (SC-105): Use this form to request the court to do something concerning your case. This form can be used before trial or after a judgment has been entered.

Forms to use after your case

Order to Appear for Examination (SC-134): You’ve won your case but can’t collect. Use this form to make the debtor appear in court and disclose his assets. For more information see Judgment Debtor Hearing.
Add Collection Cost and Interest to Your Judgment (MC-012): Have collection costs added to your judgment. Use our guide to help you fill out the form. For more information see Collection Costs.
Notice of Appeal (SC-140): A Plaintiff cannot appeal if you lose on your Plaintiff’s Claim. However, if the Defendant countersued you and you lose on the Defendant’s Claim, you can appeal. For more information, see Appeals.
Motion to Vacate Judgment and Declaration (SC-135): If you are the Defendant and lost because you didn’t appear in court, use this form to ask the court to set aside the judgment. For more information see Motion to Vacate.
Request to Correct or Cancel Judgment and Answer (SC-108): If the clerk made a clerical mistake on your judgment or the court applied the wrong law to your case, use this form to ask the court to correct or cancel the judgment.
Writ of Execution (EJ-130): Use this form to have the Sheriff�s Department collect from the Judgment Debtor�s bank account or garnish their wages. You must provide the bank account information or the place where the Judgment Debtor works. Use our guide to help you fill out the form.
